Children's National Mobile Clinic
IDEA PCS in partnership with Children's National Hospital will be hosting a mobile clinic. Students may receive immunizations or health physicals. (If a student is 17 years old or younger, a parent must accompany them via a Zoom link that Children's National will provide, or in person.)
There are several forms to fill out prior to the appointment. You may click on the link to download and fill out. Hard copies of these forms will also be provided in room 232 (students can see Ms. MaryCatherine, Ms. Monica, or Ms. Brown).
- This Immunization Form allows a student to be seen without their parent ONLY for immunization appointments.
- Students will need the Consent to Treat form signed for Immunizations.
- Please still bring your student ID or DC ID (preferred)
- This form allows a student to be seen for their physical. Children's National will send a Zoom link if you cannot make the appointment in person.
